Every report should state its definition, source date, filters, audience, and limitations. External views default to aggregate information without routine participant identities.
Enrollment, plan currency, verified milestones, referral stages, transition readiness, service timeliness, participant experience, completion, exit reasons, and consent-based follow-up may be reported with context.
A referral sent is not a service completed. Attendance is not proof of rehabilitation. Participant-reported information is not the same as source verification. Unknown remains unknown.
